Participants are required to complete the appropriate form above with their submission.

If you are chosen as a finalist, you and any co-producers must also submit a signed and completed Finalist Parent/Guardian Permission Form and a Media Release Form. A signed and completed Actor Release Form will also be required for each person appearing in your video.
